Identification and income information are needed to enroll in the sliding fee program.
The School Based Health Centers are opened year round. Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.
Billing Questions: 1 (800) 787-2568
If you have insurance including Medicaid or Nevada Check -Up, we will bill your insurance for you. Please keep your insurance card on file at the School Based Health Center your child will use. If you do not have insurance, you will not be billed.
How to be a Patient: Students from the 10 schools in the School Based Health Center service area are welcome at the Health Centers. During the first part of each school year parents will be given information about which Health Center their child can most easily access. It is best to sign a "consent to treat" form and have your child's insurance card on file at the Health Center in case your child becomes ill or has an accident at school and you cannot be reached. If you do not have insurance, your child can receive medical care at no cost to you.
Background Information: The School Based Health Centers were started by Clark County Health District in 2000. In 2005 Nevada Health Centers assumed the operation of the School Based Health Centers because federal funding became available to assist in the operational costs. School Based Health Centers are a great way for students to access health care that they may otherwise not receive.
