Sliding Fee
Program
Patients who wish to participate in our
sliding-fee-program are encouraged to ask for a Sliding Fee
Application Form which includes verification of income. In
addition, proof of address and picture identification required. In order for a patient
to qualify for the sliding fee program, the information must be
provided at the time of the visit. If the qualifying
documentation is not provided, the patient will be responsible for
100% of the cost for services rendered.

Lab
Results
Please allow a minimum of three working days for the
results of your lab tests to come back to us from the lab. If
you have an appointment, your results will be discussed at the time
of your visit. If you do not have a scheduled appointment,
your lab results will be reviewed by the clinical staff, and you
will be notified if follow up is necessary. If your lab
results are normal the patient will not receive a phone call
from the clinic.

Billing
Inquiries
For billing inquiries, please call our billing
department at (775) 887-1590 or (800) 787-2568 between the hours of 8:00 a.m. and 5:00 p.m. Monday through Friday. If you should encounter a
"recording", please leave a message and a billing person will
call you back.

Payment for
Services
Co-payments are due at the time of the office visit.
For patients who qualify for our sliding fee program,
the patient's percentage is due at the time of service. Our
clinics accept Visa, Master Card, Discover Card, Travelers Checks, Personal Checks
(with identification) and Cash.

Medical Records
Release
If you should need your medical records for any
reason in the future, you will need to sign a "Release of
Information" in our office. The release must include a
complete address of where your records need to be sent. Please
allow one week to process the copying of your chart. There may
be a charge for this service.
