Overview:
Nevada Health Centers, Inc. (NVHC) was founded in
1977 as Federally Qualified Health Centers (FQHC) and currently has 17
clinical sites throughout Nevada. The Las Vegas Outreach Clinic,
located in Clark County, first opened its doors to serve the
community with quality health care in 2000.
This clinic is the heart of NVHC’s Health Care
for the Homeless (HCH) Program and is dedicated to providing
primary health care for the people of Clark County who do not have a
place to call home. This population, which numbers somewhere between
8,000 and 10,000 men, women and children, is especially vulnerable to
illness and disease, much of it caused by exposure to the elements,
crowded living conditions found in the area’s shelters and a lack of
ready access to primary and preventative medical care.
Target Population – Las Vegas’s population
is approximately 525,000 people, and Clark County’s population is
approximately 1,600,000 people. Although estimates are conflicting, it
is presumed that there are up to 10,000 homeless individuals in the
Clark County area.
Services Provided— Family Medicine,
Women’s Health, Pediatrics, STD/HIV Education, Periodic Screenings for
Kids & Adults, Well-Child Care and Immunizations, Prenatal and Newborn
Care, Chronic Illness Management, Health Education & Case Management.
Outreach Services Provided— The HCH
Outreach Team consists of a Case Manager and a Outreach Nurse (LPN). This
team is based out of the LVOC and provides services to those people
living in the local shelters and on the streets of Clark County,
primarily in the Las Vegas valley. Medical screenings, assistance
with transportation to medical and other appointments, OTC
medications, blankets, some clothing and personal care items are
provided as a part of this team’s daily work.
